Death Certificate Responsibilities as a PCM

As the PCM for your patients, if one of the patients empaneled to you dies out in town, you as their PCM are responsible for completing that patients death certificate. You will receive an email from Decedent Affairs here at the hospital with a Waive Report from the San Diego County Medical Examiner that will be used for the Death Certificate Worksheet. Once the Worksheet is completed, Decedent Affairs will then prepare the actual Death Certificate that will be sent back to the PCM for final signature.


The patients remains will NOT be released to the family UNTIL this is completed. So PLEASE, once you receive an email from Decedent Affairs, it MUST be addressed right away!
