IM Outpatient Clinic Social Work UPDATE/ABSENCE

Unfortunately, our current IM Outpatient Clinic SW (Ms. Charysse Aviles) is being pulled to assist with covering SW needs of inpatient services for the foreseeable future. Because of this, IM Clinic will be WITHOUT an outpatient social worker during this time.


I foresee this having the greatest impact on our DME and Home Health (HH) referrals that we place often. Please see email from Peyton sent on 22AUG for more details and the Go-By's attached to that email for placing DME and HH referrals.


When in doubt on how to complete something previously completed with the assistance of outpatient SW, please reach out to your clinic Attending and your respective Nursing Team Staff for assistance.


If a SW consult is submitted via Referral 2.0, but does not meet the emergent criteria listed below, it will be returned to the provider and instead must be addressed by the IM Clinic Staff and Provider.


Emergent SW Consults that WILL be ACCEPTED: 

• Hospice

• Suspected Elder Abuse

• Domestic violence

• Child abuse and Child protective issues


We are hopeful this is temporary and appreciate your understanding.
