PCM Appointment Scheduling After Hospital Discharge

HM3 Ausley is serving as the IM Wards Discharge Planner. He is able to assist in arranging hospital discharge PRIMARY CARE follow-up appointments for patients empaneled at the surrounding branch clinics or who has a PCM at a civilian practice! HM3 is to check in with the ward teams daily (he typically works out of the Attending Blue Team Room), but please contact him via his duty phone number listed above if you need assistance.

In almost all circumstances, if the patient ALREADY has an established PCM, either at NMCSD IM, at a Branch Clinic, or with a PCM at a civilian practice, then they DO NOT need a 1-time Hospital Discharge Ward follow-up appointment in the IM Clinic. Please DO NOT place a referral for an IM Clinic 1-time ward follow-up appointment UNLESS there is a specific and/or urgent clinical question/item that needs to be addressed that you believe will not be able to be addressed by their PCM in the appropriate time OR the patient has no PCM at all. If you have questions about if a 1-time IM Ward follow-up referral is needed, please ask your resident, attending, or the IM CoRs.

Again, at this time, he is ONLY assisting with scheduling PRIMARY CARE follow-up appointments with patients empaneled at a branch clinic OR have a PCM at a civilian practice. He can also assist with faxing the physician discharge summaries to civilian PCMs following discharge.

LIMITATIONS:

1. For patients who are empaneled at our NMCSD IM Clinic, please continue to message the Blue Team or Gold Team scheduler/nursing pools via Genesis. For now, HM3 will not be scheduling these patients.

2. HM3 is unable to schedule specialty/sub-specialty care appointments, either within our hospital system or at civilian specialists. Please provide the patient with the appropriate NMCSD Clinic phone number, the number for Referral Management if placing a referral at time of discharge, or instruct them to contact their Civilian Sub-specialist provider in order to schedule a hospital discharge follow-up appointment.
